5-Whys analysis would generally be used in the day to day ops arena where the circumstances of the "failure" are relatively easily understood.
The Kelvin TOP-SET method is a more formalised and structured approach to root cause analysis. This approach would typically be used when an investigation has already been carried out for a more serious event. The investigation report and evidence gathered would typically be handed over to an independent team to lead. The circumstances of this event would be less well understood and a greater level of teasing out is required.
The TapRooT method is a more complete tool. Typically this would be employed on more complex or severe incidents. The complete method employs not only the analysis tool but investigation/interview too. This lends itself well to independent investigation / data gathering and analysis of complex incidents where consequences were, or could have been, severe.
